Severe Pneumocystis Pneumonia Associated with Ulcerative Colitis

A 56-year-old woman with a 9-month history of ulcerative colitis (UC) who had been treated prednisolone (PSL) 40 mg/day and azathioprinefor 2 months was referred to our hospital because fever dyspnea. Computed tomography (CT) the chest revealed diffuse ground-glass opacities in both lungs consolidation lower lobes. PCR patient's bronchoalveolar lavage fluid positive for pneumocystis jirovecii, she diagnosed pneumonia (PCP). The patient developed refractory respiratory failure that required invasive mechanical ventilation. After sulfamethoxazole-trimethoprim administration methyl pulse therapy, her clinical condition improved dramatically. Few reports are available on severe PCP associated UC. It is important keep mind possibility complication by UC patients.
